You may able to complete a request for a Certified Address online and/or by telephone.

If you have Adobe Acrobat Reader, you may download the Request for Certified Address Form, complete it using the Reader program, and email it to us at: addressing@knoxmpc.org

Or, you may download the form, print and complete it by hand, and return it by fax to: 865-215-2237.

Once the form is returned to the Addressing Department, we will contact you with information on completing the certification process.
